New Patients/Pacientes Nuevos

In order to better service our patients, we have free covered parking behind our building, which means that you don't have to bother looking for parking spaces or paying exorbitant parking/meter fees.

The Institute is a full service Cardiology and Internal Medicine facility, which means that you won't have to make separate appointments and drive somewhere else for bloodwork, urinalysis, and most tests.
We have extended office hours, which means that you don't have to take off from work to keep your appointment.

You should bring the following items to your appointment:
1) Photo I.D.
2) List of prescription meds you are taking.
3) List of questions for the doctor.
4) Referral authorization (if needed).
5) Insurance Card.

So that we can shorten the time our new patients wait to be seen because they must fill out patient information sheets, please download the initial forms here. Have them filled out and signed and bring them with you to the first visit,to avoid delays on arrival.

Para mejorar los servicios de nuestros pacientes, nosotros tenemos parqueaderos cubiertos gratuitos detrás de nuestro edificio, lo cual significa que ustedes no tienen que preocuparse por buscar espacio para parqueo o pagar tarifas excesivas en parquímetros.

El Instituto tiene un servicio completo de Cardiología y Medicina en el mismo edificio, lo cual significa que ustedes no tendran que solicitar citas de atencion separadas y dirigirser hacia algun lugar para chequeos sanguíneos, analisis de orina y otros chequeos.
Nosotros tenemos horarios de oficina extendidos, lo cual significa que usted no tienen que salir temprano y perder tiempo de trabajo para asistir a sus citas.

Usted debería llevar lo siguiente para sus citas:
1) Identificacion con foto.
2) Lista de las medicinas que ud esta tomando.
3) Lista de preguntas para el doctor.
4) Referido de autorizacion(si es necesario).
5) Tarjeta de su companía Aseguradora.

Nosotros podemos disminuir el tiempo para ver Pacientes Nuevos porque ellos pueden traer a nuestra oficina la hoja de información del paciente diligenciada previamente. Por favor abrir el archivo de formato, Haciendo un clic. Llénelo, fírmelo y traigalo con Usted en su primera visita, para evitar demoras al llegar.
